Patient Registration Forms

Harter Physical Therapy in Goddard, Kansas welcomes you to our practice. Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit, along with any diagnostic films or other test results that may have been ordered or performed by another physician. Completing the forms prior to your visit will help speed up the new patient registration process.

What to Expect

We hope the following will help prepare you for your first day of physical therapy with us:

  • Please arrive 15 – 30 minutes prior to your scheduled appointment time. This will provide adequate time to complete necessary pre-evaluation paperwork, and ensure that our doctor of physical therapy (DPT) will be able to spend the entire scheduled treatment session with you.
  • Day 1 will be primarily for examination, and generally lasts approximately one hour, allowing us sufficient time to complete an extensive evaluation. The evaluation includes an initial discussion aimed to obtain a history of your condition while also getting to know more about you, your activities, and your goals. Following the discussion, an objective exam will be performed, which includes joint tests, muscle tests, and other special tests to isolate and assess the integrity of specific structures in the body. This allows us to determine which structures may be at fault, or contributing to your symptoms. Your specific pathoanatomical diagnosis will be thoroughly explained to you and an individualized plan of care will be discussed, created, and initiated at that time.
  • We allow the time for and encourage our patients to ask questions. Our goal is to provide a patient-centered interaction that promotes patient understanding and the empowerment to achieve their goals.
  • Wear comfortable clothes. Many of our patients prefer to wear athletic or loose-fitting clothes. Gowns and athletic shorts are also available at our clinic, if you prefer.
  • Relax! We move at your pace. Our goal is to help you in any way that we can. Your therapy session will be designed specifically for you and your needs.

What to Bring

Please bring the following items to your initial appointment:

  • Patient forms - Please try to fill these out before you come to your first appointment, and bring them with you or fax to (316) 550-6215. If you have any questions while filling out the patient forms, please do not hesitate to give us a call. While filling out the forms ahead of time is not a requirement, it will decrease the amount of time required to fill out all necessary forms once at HPT clinic.
  • Primary insurance card (we accept most insurances).
  • Secondary insurance card, if applicable

  • Any diagnostic imaging you may have relating to your condition, such as X-ray, CT Scan, MRI, etc.
